Reaction patterns by class

  • Most antiarrhythmics are proarrhythmic - the reaction they cause is often the same class of problem they treat
  • Class-specific patterns
    • Amiodarone: multi-organ toxicity (thyroid, lung, liver, cornea, skin) - the classic "everything drug"
    • Flecainide (Class Ic): proarrhythmia in structural/ischaemic heart disease - contraindicated
    • Sotalol: QT prolongation -> torsades (beta-blocker + Class III combined effect)
