- ATRA (all-trans retinoic acid) treats acute promyelocytic leukaemia (APL, AML M3) by forcing terminal differentiation of the malignant promyelocyte clone (targets the PML-RARA fusion product)
- Differentiation syndrome: massively differentiating leukocytes release inflammatory cytokines -> capillary leak -> multi-organ involvement
- Occurs in ~15-25% of APL patients started on ATRA (or arsenic trioxide), typically within the first 2-3 weeks
