- Some antibiotics are chosen (or added) for an antitoxin/immunomodulatory effect independent of killing bacteria -- exam tests recognising this non-bactericidal rationale
- Clindamycin: inhibits 50S ribosomal subunit (protein synthesis) -> at the mechanistic level this suppresses exotoxin production (e.g. streptococcal/staphylococcal toxins) even at sub-bactericidal concentrations
- Used adjunctively in toxin-mediated disease: necrotising fasciitis, toxic shock syndrome, severe streptococcal/staphylococcal infection
