- QTc >500ms or increase >60ms from baseline - high torsades risk
- Polymorphic VT / torsades de pointes
- Wide-complex tachycardia after TCA or flecainide overdose
- Bradyarrhythmia with hypotension (beta-blocker/CCB/digoxin)
- Syncope temporally linked to a new QT-prolonging drug
