Red flags
- Haematemesis/melaena in a patient on NSAID, antiplatelet, or anticoagulant
- Shock index >1, ongoing bleeding despite resuscitation
- Bleeding on combined antiplatelet + anticoagulant + NSAID (highest-risk combination)
- Elderly, H. pylori positive, prior ulcer - highest-risk group for NSAID bleeding
