- Hypotension with bradycardia - beta-blocker/CCB/digoxin toxicity
- Hypotension with wide QRS tachycardia - TCA/sodium channel blocker
- Urticaria, angioedema, wheeze - anaphylaxis
- Refractory to fluid bolus
- Combined beta-blocker + CCB ingestion (more lethal than either alone)
- Hypotension + reduced consciousness + hypoglycaemia - beta-blocker overlap
