- Rigidity + fever + autonomic instability + altered consciousness - NMS
- Trismus, torticollis, oculogyric crisis in a young patient after antiemetic/antipsychotic - acute dystonia (airway risk if laryngeal)
- Hyperreflexia + clonus + fever + agitation - serotonin syndrome
- Sudden severe rigidity with hyperthermia - consider malignant hyperthermia (anaesthetic trigger) as a differential
