- Life-threatening autonomic emergency in patients with spinal cord injury at or above T6 - an unregulated sympathetic surge below the level of injury, triggered by a noxious stimulus
- Presents with severe paroxysmal hypertension, often with bradycardia, pounding headache, and flushing/sweating above the lesion with pallor below it
