- Bone metabolism disorders reflect disturbances in the bone remodelling balance between osteoblast-driven formation and osteoclast-driven resorption, or in mineralisation itself - osteoporosis (reduced bone mass/quality with normal mineralisation) and osteomalacia (impaired mineralisation, classically from vitamin D deficiency) are the two dominant categories and require different investigation and treatment despite both increasing fracture risk
- Distinguishing the two matters clinically: osteomalacia is often reversible with vitamin D/calcium repletion, while osteoporosis requires bone-specific pharmacotherapy
