- Troponin = regulatory complex on the thin filament (troponin T, I, C) - cardiac isoforms of T and I are myocardium-specific, unlike CK-MB or myoglobin
- Released on cardiomyocyte membrane injury/necrosis, not exclusively ischaemic infarction
- Detects damage of any cause - the assay is sensitive and specific for myocyte injury, not specific for the mechanism ("troponin-positive, not necessarily ACS")
