Diagnostic and screening tests - likelihood ratios

Definition

  • Likelihood ratios combine sensitivity and specificity into a single number describing how much a specific test result (positive or negative) shifts the odds of disease from pre-test to post-test probability - unlike predictive values, likelihood ratios are prevalence-independent (fixed test properties), making them arguably the most clinically useful and flexible way to interpret a test result across different clinical contexts
  • Increasingly emphasised in clinical teaching as the preferred method for applying test results to individual patient probability estimation, given their combination of the fixed-property advantage of sensitivity/specificity with direct clinical applicability
