Pancreatitis
Description
Acute pancreatitis - revised Atlanta classification
| Severity | Definition |
|---|---|
| Mild | No organ failure, no local or systemic complications |
| Moderately severe | Transient organ failure (<48 h) and/or local/systemic complications |
| Severe | Persistent organ failure >48 h (single or multiple) |
- Two phases: early (<1 week, SIRS-driven) and late (>1 week, complication- and infection-driven)
- Morphology: interstitial oedematous vs necrotising (~10-20%)
Local collections - the naming matters
| Timing | Non-necrotic | Necrotic |
|---|---|---|
| <4 weeks | Acute peripancreatic fluid collection | Acute necrotic collection |
| >4 weeks (encapsulated) | Pseudocyst | Walled-off necrosis |
Chronic pancreatitis
- Irreversible fibrosis, calcification, duct distortion -> exocrine then endocrine insufficiency + chronic pain
