- Gene therapy replaces/supplements a defective gene product -- applicability differs fundamentally by inheritance pattern, and this is the tested concept
- Autosomal recessive / X-linked recessive (loss-of-function) disease: adding a single functional copy of the gene is often sufficient to restore adequate protein function -- these conditions are the most amenable to straightforward gene-addition therapy
- Autosomal dominant disease caused by a gain-of-function or dominant-negative mutation: simply adding a normal gene copy does not help, because the mutant allele's product continues to interfere -- these need gene silencing/editing (targeting the mutant allele itself), not gene addition
