  • Hypothalamus: GHRH (stimulates) vs somatostatin (inhibits) -> anterior pituitary GH -> acts directly AND via IGF-1 (mainly hepatic)
  • GH has direct metabolic effects (lipolysis, anti-insulin/diabetogenic, protein synthesis) and indirect growth-promoting effects mediated by IGF-1
  • Negative feedback: IGF-1 inhibits both GHRH and GH (long loop); GH itself stimulates somatostatin (short loop)
