- Where did it come from? (site-of-origin markers in CUP)
- What will it respond to? (predictive - ER, PR, HER2, PD-L1)
- Ki-67 - a nuclear protein expressed in G1, S, G2 and M but absent in G0
- A direct proliferation index = % of nuclei staining
- Not lineage-specific; it grades, it does not diagnose

- Cytokeratin -> epithelial (carcinoma)
- LCA/CD45 -> lymphoid
- S100, SOX10, HMB-45, Melan-A -> melanocytic
- Vimentin, desmin, SMA -> mesenchymal
- Chromogranin, synaptophysin, INSM1, CD56 -> neuroendocrine
