  • Elevated blood cell counts (any lineage) require distinguishing a reactive/secondary cause (infection, inflammation, hypoxia-driven erythropoietin excess) from a primary clonal haematological process (myeloproliferative neoplasm) - the distinction determines whether investigation should focus on the underlying trigger or on bone marrow/molecular studies
  • Marked or persistent elevation, especially with more than one lineage affected or in the absence of an obvious reactive trigger, raises suspicion for an underlying myeloproliferative neoplasm
