Nutrition - refeeding management in chronically undernourished patients and in the case of starvation

Definition

  • Refeeding syndrome is a potentially fatal metabolic derangement (hypophosphataemia, hypokalaemia, hypomagnesaemia, and fluid overload) precipitated by reintroducing nutrition too rapidly to a chronically malnourished or starved patient - requiring active anticipation and cautious, staged nutritional reintroduction rather than standard full-rate feeding
  • A well-recognised risk in any significantly malnourished patient (chronic starvation, eating disorders, chronic alcohol excess, prolonged fasting/NPO status, oncology patients with significant weight loss) being started on nutritional support (oral, enteral, or parenteral)
