Overview
- Hepatic impairment reduces drug clearance for hepatically-metabolised agents through reduced enzymatic metabolic capacity, reduced hepatic blood flow (affecting high-extraction drugs particularly), and reduced protein synthesis (increasing free fraction of highly protein-bound drugs) - unlike renal impairment, there is no single validated biomarker (equivalent to eGFR) providing a direct, standardised measure of hepatic drug-clearing capacity
