- Pneumoconiosis = lung fibrosis from inhaled inorganic dust, mechanism is macrophage-mediated, not direct toxicity
- Alveolar macrophages phagocytose deposited particles -> unable to fully degrade them -> chronic macrophage activation -> release of cytokines/fibrogenic mediators (TNF, TGF-beta) -> fibroblast proliferation and collagen deposition
- Silica is directly cytotoxic to macrophages as well (membranolytic) -> macrophage death releases more mediators -> self-perpetuating inflammation even after exposure ceases
