The principle
- Efficient gas exchange requires ventilation (airflow to alveoli) and perfusion (blood flow to alveolar capillaries) to be matched at the regional level - mismatch, not simply "low ventilation" or "low perfusion" alone, is the dominant mechanism of hypoxaemia in most lung disease
- V/Q mismatch is the commonest cause of hypoxaemia encountered clinically, more common than true shunt or diffusion limitation
