Red flags

  • Hot, swollen single joint with fever - septic arthritis until excluded, needs urgent aspiration
  • Calf swelling + pain + risk factors - DVT until excluded
  • Compartment syndrome features (pain out of proportion, pain on passive stretch, paraesthesia) - surgical emergency
  • Rapidly spreading erythema/swelling with systemic toxicity - necrotising fasciitis
  • Swelling + neurovascular compromise distally
