Vasopressin receptor antagonists (tolvaptan) - mechanism
Core concept
- Vaptans block the V2 receptor on the collecting duct principal cell
- Normally: ADH -> V2 (Gs) -> cAMP -> aquaporin-2 insertion into the apical membrane -> water reabsorption
- Blockade -> AQP2 removed -> free water excretion without solute loss = aquaresis
- Aquaresis, not diuresis - urine osmolality falls, sodium and potassium excretion barely change
- -> serum sodium rises because water is lost in excess of solute
- Contrast with a loop diuretic (loses Na and water) and with fluid restriction (limits intake)
- Tolvaptan - oral, selective V2; conivaptan - IV, mixed V1a/V2
