The COWS

  • The Clinical Opiate Withdrawal Scale (COWS) is a validated scoring tool assessing opioid withdrawal severity across observed and reported symptoms (pupil size, sweating, restlessness, GI symptoms, and others) - used both to guide symptom-triggered treatment and, importantly, to confirm adequate withdrawal severity before initiating buprenorphine (given its precipitated withdrawal risk if started too early)
  • Widely used in both acute withdrawal management settings and specifically in opioid agonist treatment induction, where confirming an adequate COWS score before the first buprenorphine dose is a standard, safety-relevant step
