Positive predictive value calculation from prevalence, sensitivity, specificity (Bayes theorem)
Core concept
- *Sens/spec are properties of the test. PPV/NPV are properties of the test in this population.*
```
Disease + Disease -
Test + TP FP
Test - FN TN
```
- Sens = TP/(TP+FN) - read down the disease-positive column
- Spec = TN/(TN+FP) - down the disease-negative column
- PPV = TP/(TP+FP) - read across the test-positive row
- NPV = TN/(TN+FN)
Bayes form (when you are given percentages, not counts)
```
PPV = (sens x prev) / [ (sens x prev) + (1 - spec) x (1 - prev) ]
```
- numerator = true positives
- right-hand term of denominator = false positives
- dec prevalence -> dec PPV, however good the test
